What Is a Urologist?

A urologist is a physician who has completed a certified residency training program. Board certified urologists have successfully completed oral and written exams conducted by the American Board of Urology.

To Make An Appointment...

St. Luke’s Mountain States Urology is open to serve you Monday through Friday, from 9 am to 5 pm. Patients are seen by appointment. Please notify us if you will be late; you may be asked to reschedule. If you are a new patient, you will be mailed paperwork to complete prior to your appointment. We ask that you bring your insurance card with you to each visit and notify us of any changes in your address, phone number, and/or insurance information. Medicare, Medicaid, and most private insurances are accepted. To schedule an appointment at one of our clinics, please call:

Cancellations

Please notify us at least 24 hours in advance if you will not be able to make your scheduled appointment. This will enable us to serve all patients in a timely manner.

Prescription Refills
