Boise School District Screenings
The District and St. Luke's Occupational Health clinic screening includes biometric measures for blood pressure, height, weight and waist circumference as well as lab tests; cholesterol, triglycerides, HDL, LDL and glucose. Results will be mailed to participants within 2-3 weeks.
What are the 3 requirements needed to be eligible for the 2014/15 Wellness Plan Option?
- Complete the on-line Personal Health Assessment (PHA) at www.wellsteps.com/boiseschools.
- Complete a "simple biometric health screening" at a District location or with your own physician.
- Complete 1 approved Wellness Activity focused on healthy living.
The activity options will include completing either a WellSteps group campaign focused on healthy living or an approved personal wellness activity. The deadline date for completing all 3 requirements is Friday, March 7, 2014.

- Remember to FAST - no food or drink except water - for 8 to 12 hours prior to your appointment. You may take your prescription medications as usual.
