Boise School District Screenings

Description:
The District and St. Luke’s Occupational Health clinic screening includes biometric measures for blood pressure, height, weight and waist circumference as well as laboratory tests for cholesterol, triglycerides, HDL, LDL and glucose. Results will be mailed to your home address.

To opt-in to Wellness Plan 2013/14, do two things by January 31, 2013
1. Complete the WellSteps Personal Health Assessment at https://www.wellsteps.com/boiseschools AND
2. Complete a biometric health screening at a District location or with your own physician.

IMPORTANT! PLEASE READ:

  • Remember to FAST – no food or drink except water – for 8 to 12 hours prior to your appointment. You may take your prescription medications as usual.
  • IMPORTANT - FOR THOSE HAVING ADDITIONAL LAB TESTS WITH A DOCTOR'S ORDER
    Additional tests are available and require you to bring a physician's lab order with appropriate billing codes, a photo copy of your Regence insurance card, and a completed registration form included with the brochure recently mailed to your home.
