|Russell Mayes, DO is board certified in otolaryngology and facial plastic and reconstructive surgery. He earned his medical degree from Midwestern University, Arizona College of Osteopathic Medicine and performed his residency at Michigan State University in East Lansing.|
Julian Nicholson, MD is board certified in otolaryngology, head and neck surgery, and facial plastic and reconstructive surgery, and is a member and fellow of the American College of Surgeons. He earned his medical degree from the Medical College of Georgia in Augusta and performed his residency at Geisinger Medical Center in Danville, Pennsylvania.
Kylan Peterson, DO is board certified in otolaryngology and practices ear, nose, throat, and facial plastic surgery. He earned his medical degree from A.T. Still University - Kirksville College of Medicine and completed his internship and residency through Michigan State University in Grand Blanc, where he served as chief resident. Dr. Peterson is fluent in Spanish. (Se habla espaņol.)
Cortney Mitchell, AuD, CCC-A, F-AAA has professional interests in audiologic diagnostic testing and amplification for adults and children, auditory evoked potential (ABR) testing, and vestibular diagnostic testing. Dr. Mitchell earned her bachelor's degree in communicative disorders from Utah State University and her clinical doctorate degree in audiology from the University of Utah.
Sonda LaDeaux, MS, CCCA is a certified audiologist whose experience includes hearing testing for all ages, balance and dizzy testing and treatment, hearing aid selection, programming, trouble shooting, and minor repairs for children and adults. She earned her bachelor's degree in audiology from Brigham Young University and master's degree in audiology from Utah State University.
Jay C. Lloyd, MS, CCC has been a practicing audiologist for 40 years, working with ear, nose and throat physicians his entire career. He has a wide range of experience in all aspects of audiology, currently working with patients in the selection of hearing aids and assisting them with the use of amplification. Jay earned his bachelor's degree from Idaho State University, completed his master's degree in speech pathology and audiology, and holds a certificate of clinical competence in audiology.
We try to stay on time, as a courtesy to you and others. Since paperwork may take a few minutes, please arrive early. As emergencies do occur, there may be occasions when your appointment time will be delayed. We will always try to keep you informed in the event of a delay, and will be happy to reschedule your appointment, if you prefer. We ask that you respect the appointment time we have reserved for you, and call 24 hours ahead to cancel or change appointments so that others may be helped during that time.
Communication with your provider is extremely important. It may be helpful for you to write down questions for your doctor or nurse practitioner prior to your visit. Bring a list of medications and the dosages, or bring the bottles with you.
Make sure you understand any instructions you are given. If clarification is needed, review your instructions with the provider or medical assistant before you leave. If you aren’t doing well after your visit, we want to hear from you. Our goal is to improve your health.
We accept Medicare, in-state Medicaid, Tricare, and most private insurance plans. We ask that you bring your insurance card with you to each visit and notify us of any changes in your address, phone number, or insurance information.
Talk with your provider at each visit to be sure your prescriptions are up to date. For refills, contact your pharmacy. Your pharmacy will then contact our office for verification and approval. For patient safety, narcotics cannot be refilled outside of office hours. Talk with your provider before your prescription runs out, and please allow 24-48 hours for refills.
