What is Family Medicine?
Family Medicine doctors are trained to care for patients of all ages, from infancy through the end of life. We promote the concept of a Medical Home, emphasizing a continuous relationship with a personal physician coordinating care for both wellness and illness. We believe that knowing you over many years, and knowing your family, helps us understand how your health care impacts your life.
St. Luke’s Family Medicine physicians diagnose and treat most medical issues, from infections and sprains to diabetes and asthma. We also specialize in preventive care to help you create and maintain a healthy lifestyle. And when your condition requires a specialist, we can coordinate your care with other physicians and can help you navigate your way through today’s health care.

Our Medical Providers
Michael K. Maier, MD is board certified in family medicine with added certification in sports medicine. He completed his undergraduate degree at Boise State University in 1979 and earned his medical degree from the University of Washington in Seattle in 1983. He completed his residency in 1986, at the Family Practice Residency of Idaho in Boise, where he also served as faculty. Dr. Maier began private practice at Mountain View Medical Center in 1987.
Eric L. Maier, MD is board certified in family medicine with added certification in sports medicine. He completed his undergraduate degree at Boise State University in 1981 and earned his medical degree from the University of Utah in Salt Lake City in 1985. Dr. Maier completed his residency at the Family Practice Residency of Idaho in Boise in 1988, and began private practice at Mountain View Medical Center that same year.
Mark C. Johnson, MD is board certified in family medicine. He completed his undergraduate degree at the University of Washington in 1981 and earned his medical degree from St. Louis University in St. Louis, Missouri in 1986. He completed his residency at the Family Practice Residency of Idaho in Boise, where he also served as faculty. Dr. Johnson began private practice in 1991.
Gertjan (Jerry) Mulder, MD is board certified in family medicine with added certification in sports medicine. He completed his undergraduate degree at the University of Washington in Seattle in 1980, where he also earned his medical degree in 1984. After residency, Dr. Mulder served as faculty and associate director at the Family Practice Residency of Idaho until 1995, when he began private practice at Mountain View Medical Center.
Russell M. Kocemba, MD is board certified in family medicine. He completed his undergraduate degree at the University of St. Thomas in Saint Paul, Minnesota in 1991 and earned his medical degree from the University of Minnesota in 1995. He completed his residency at the Family Practice Residency of Idaho in Boise in 1998. He began private practice at Mountain View Medical Center in 2000.
R.J. Axtell, MD is board certified in family medicine. He completed his undergraduate degree at the College of Idaho in Caldwell in 2000 and earned his medical degree from the University of Washington in Seattle in 2004. He completed his residency at Family Medicine of Southwest Washington in Vancouver, and joined St. Luke's Clinic - Mountain View Medical Center in 2010.
Your Appointment
We try to stay on time, as a courtesy to you and others. Since paperwork may take a few minutes, please arrive early. As emergencies do occur, there may be occasions when your appointment time will be delayed. We will always try to keep you informed in the event of a delay, and will be happy to reschedule your appointment, if you prefer. We ask that you respect the appointment time we have reserved for you, and call 24 hours ahead to cancel or change appointments so that others may be helped during that time.
Communication with your provider is extremely important. It may be helpful for you to write down questions for your doctor or nurse practitioner prior to your visit. Bring a list of medications and the dosages, or bring the bottles with you.
Make sure you understand any instructions you are given. If clarification is needed, review your instructions with the provider or medical assistant before you leave. If you aren’t doing well after your visit, we want to hear from you. Our goal is to improve your health.
Insurance Plans
We accept most insurance plans, including Medicare, in-state Medicaid, Tricare, Blue Cross/Blue Shield, and others.
Medication Refills
Talk with your provider at each visit to be sure your prescriptions are up to date. For refills, contact your pharmacy. Your pharmacy will then contact our office for verification and approval. For patient safety, narcotics cannot be refilled outside of office hours. Talk with your provider before your prescription runs out, and please allow 48 hours for refills.
