Our Medical Providers


David C. Allen, MD David C. Allen, MD is board certified in obstetrics and gynecology and has been in practice in Twin Falls since 1994. He earned his associate degree in biology from Ricks College after serving a mission for the LDS church in Korea. He earned his bachelor's degree in zoology from Brigham Young University in 1983 and his medical degree from the University of Texas Medical Branch in Galveston. Dr. Allen completed his four-year obstetrics/gynecology residency at the John Peter Smith Hospital in Fort Worth in 1994.
Marc T. Astin, MD Marc T. Astin, MD is board certified in obstetrics and gynecology and has been in practice in the Magic Valley since 1985. He earned his bachelor's degree in chemistry in 1976 from the University of Utah, where he also earned his medical degree. Dr. Astin completed his four-year obstetrics/gynecology residency at LDS Hospital, University of Utah Medical Center in 1985.
Darren W. Coleman, MD Darren W. Coleman, MD is board certified in obstetrics and gynecology and has been in practice in Twin Falls since 1999. He earned his bachelor's degree from Brigham Young University in 1991 and his medical degree from the University of Utah. Dr. Coleman completed his four-year obstetrics/gynecology residency at Indiana University Medical Center in Indianapolis in 1999.
E. Monte Crandall, MD E. Monte Crandall, MD is board certified in obstetrics and gynecology. He earned his bachelor's degree in psychology in 1977 from the University of Utah, where he also earned his medical degree. Dr. Crandall completed his four-year obstetrics/gynecology residency at the University of Iowa Hospital and Clinics in Iowa City in 1985.
Laura Fall, MD Laura Fall, MD is board certified in family practice. She earned her bachelor's degree in biology/Spanish/pre-medicine with a concentration in chemistry from Augustana College in Rock Island, Illinois in 1984. She earned her medical degree from the University of Health Sciences/The Chicago Medical School and completed her family practice internship and residency in 1992 at Carbondale Memorial Hospital (a program affiliated with The Southern Illinois University Medical School), where she also served as chief resident. Dr. Fall is fluent in Spanish.
Elizabeth Johnson, MD Elizabeth Johnson, MD is board certified in family medicine. She earned her medical degree from the University of Colorado School of Medicine and completed her family medicine residency at North Colorado Medical Center.
Matthew H. Packham, MD Matthew H. Packham, MD is board certified in obstetrics and gynecology. He earned his bachelor's degree in human biology from the University of Utah in 1996 and his medical degree from the University of Washington School of Medicine. He completed his four-year obstetrics/gynecology residency at the San Antonio Uniformed Services Health Education Consortium in 2005. Dr. Packham was part of the U.S. Army 25th Infantry Division and served in Iraq in 2008-2009. He practiced for four years in Fairbanks, Alaska before joining St. Luke's Clinic - Obstetrics and Gynecology in 2009.
Donald E. Smith, MD Donald E. Smith, MD is board certified in obstetrics and gynecology. He earned his bachelor's degree in psychologyin 1980 from the University of Utah, where he laso earned his medical degree. Dr. Smith completed his four-year obstetrics/gynecology residency at Mayo Clinic in Rochester in 1988.
Kari Brose, PA-C Kari Brose, PA-C is a certified physician assistant. She earned her bachelor's degree in zoology in 1995 and physician assistant studies degree in 2003, both from Idaho State University. She worked for Family Health Services for four years before joining St. Luke's Clinic in 2007.
Mindy Brown, PA-C Mindy Brown, PA-C is a certified physician assistant. She earned her bachelor's degree from the University of Nevada, Reno in 2004 and her master's degree in physician assistant studies from Idaho State University in 2006.
Maria Rosario Smith, NP Maria Rosario Smith, NP is a certified nurse practitioner. She earned her bachelor's degree from Idaho State University in 2008 and her certified nurse practitioner degree from the University of Southern Indiana in 2013.
Sharon Thueson, PA-C Sharon Thueson, PA-C is a certified physician assistant. She earned her bachelor's degree in psychology from the University of Idaho and completed her physician assistant training at Pennsylvania State University in 1981. She worked in family medicine for 24 years before joining St. Luke's Clinic in 2006.

Your Appointment
We try to stay on time, as a courtesy to you and others. Since paperwork may take a few minutes, please arrive early. As emergencies do occur, there may be occasions when your appointment time will be delayed. We will always try to keep you informed in the event of a delay, and will be happy to reschedule your appointment, if you prefer. We ask that you respect the appointment time we have reserved for you, and call 24 hours ahead to cancel or change appointments so that others may be helped during that time.

Communication with your provider is extremely important. It may be helpful for you to write down questions for your doctor or nurse practitioner prior to your visit. Bring a list of medications and the dosages, or bring the bottles with you.

Make sure you understand any instructions you are given. If clarification is needed, review your instructions with the provider or medical assistant before you leave. If you aren’t doing well after your visit, we want to hear from you. Our goal is to improve your health.


Insurance Plans
We accept most insurance plans, including Medicare, in-state Medicaid, Tricare, Blue Cross/Blue Shield, and others.


Medication Refills
Talk with your provider at each visit to be sure your prescriptions are up to date. For refills, contact your pharmacy. Your pharmacy will then contact our office for verification and approval. For patient safety, narcotics cannot be refilled outside of office hours. Talk with your provider before your prescription runs out, and please allow 48 hours for refills.
