Our Medical Providers


David C. Allen, MD David C. Allen, MD earned his associate degree in biology from Ricks College in Rexburg, Idaho after serving a mission for the LDS church in Korea. He earned his Bachelor of Science degree in zoology from Brigham Young University in Provo, Utah in 1983, and his medical degree from the University of Texas Medical Branch in Galveston. He completed his four-year obstetrics/gynecology residency at the John Peter Smith Hospital in Fort Worth, Texas in 1994 and has practiced in Twin Falls ever since. Dr. Allen is board certified in obstetrics and gynecology.
Marc T. Astin, MD Marc T. Astin, MD earned his Bachelor of Science degree in chemistry from the University of Utah in Salt Lake City in 1976 and his medical degree from the University of Utah College of Medicine. He completed his four-year obstetrics/gynecology residency at LDS Hospital, University of Utah Medical Center in 1985. Dr. Astin is board certified in obstetrics and gynecology. He has practiced in the Magic Valley since 1985 and is currently chairman of the Obstetrics/Gynecology department at St. Luke’s Magic Valley Medical Center, a position he has held since 2009.
Darren W. Coleman, MD Darren W. Coleman, MD earned his Bachelor of Science degree from Brigham Young University in in Provo, Utah in 1991 and his medical degree from the University of Utah College of Medicine in Salt Lake City. He completed his four-year obstetrics/gynecology residency at Indiana University Medical Center in Indianapolis in 1999 and joined his current practice that same year. Dr. Coleman is board certified in obstetrics and gynecology.
E. Monte Crandall, MD E. Monte Crandall, MD earned his Bachelor of Science degree in psychology from the University of Utah in Salt Lake City in 1977 and his medical degree from the University of Utah College of Medicine. He completed his four-year obstetrics/gynecology residency at the University of Iowa Hospital and Clinics in Iowa City in 1985 before moving to Twin Falls. Dr. Crandall is board certified in obstetrics and gynecology.
Laura Fall, MD Laura Fall, MD earned her Bachelor of Arts degree in biology/Spanish/pre-medicine with a concentration in chemistry from Augustana College in Rock Island, Illinois in 1984. She earned her medical degree from the University of Health Sciences/The Chicago Medical School and completed her family practice internship and residency in 1992 at Carbondale Memorial Hospital (a program affiliated with The Southern Illinois University Medical School), where she also served as chief resident. Dr. Fall is board certified in family practice and fluent in Spanish.
Matthew H. Packham, MD Matthew H. Packham, MD earned his Bachelor of Science degree in human biology from the University of Utah in Salt Lake City in 1996. He earned his medical degree from the University of Washington School of Medicine in Seattle and completed his four-year obstetrics/gynecology residency at the San Antonio Uniformed Services Health Education Consortium in 2005. Dr. Packham was part of the US Army 25th Infantry Division and served in Iraq in 2008-2009. He practiced for four years in Fairbanks, Alaska before joining St. Luke’s Clinic – Obstetrics and Gynecology in 2009. He is board certified in obstetrics and gynecology.
Donald E. Smith, MD Donald E. Smith, MD earned his Bachelor of Arts degree in psychology from the University of Utah in Salt Lake City in 1980 and his medical degree from the University of Utah College of Medicine. He completed his four-year obstetrics/gynecology residency at the Mayo Clinic in Rochester, Minnesota in 1988. Dr. Smith is board certified in obstetrics and gynecology
Kari Brose, PA-C Kari Brose, PA-C earned her bachelor’s degree in zoology in 1995 and bachelor’s degree in physician assistant studies in 2003, both from Idaho State University in Pocatello. She is certified as a physician assistant, and worked for Family Health Services for four years before joining St. Luke’s Clinic in 2007.
Mindy Brown, PA-C Mindy Brown, PA-C earned her bachelor’s degree from the University of Reno in 2004 and her master’s degree in physician assistant studies from Idaho State University in Pocatello in 2006. Mindy is certified as a physician assistant
Sharon Thueson, PA-C Sharon Thueson, PA-C earned a bachelor's degree in psychology from the University of Idaho in Moscow and completed her physician assistant training at Pennsylvania State University in 1981. She worked as a Family Practice PA for 24 years before joining our practice in 2006.

Your Appointment
We try to stay on time, as a courtesy to you and others. Since paperwork may take a few minutes, please arrive early. As emergencies do occur, there may be occasions when your appointment time will be delayed. We will always try to keep you informed in the event of a delay, and will be happy to reschedule your appointment, if you prefer. We ask that you respect the appointment time we have reserved for you, and call 24 hours ahead to cancel or change appointments so that others may be helped during that time.

Communication with your provider is extremely important. It may be helpful for you to write down questions for your doctor or nurse practitioner prior to your visit. Bring a list of medications and the dosages, or bring the bottles with you.

Make sure you understand any instructions you are given. If clarification is needed, review your instructions with the provider or medical assistant before you leave. If you aren’t doing well after your visit, we want to hear from you. Our goal is to improve your health.


Insurance Plans
We accept most insurance plans, including Medicare, in-state Medicaid, Tricare, Blue Cross/Blue Shield, and others.


Medication Refills
Talk with your provider at each visit to be sure your prescriptions are up to date. For refills, contact your pharmacy. Your pharmacy will then contact our office for verification and approval. For patient safety, narcotics cannot be refilled outside of office hours. Talk with your provider before your prescription runs out, and please allow 48 hours for refills.
